- Owner/Builder Affidavit (if applicable)
- Two (2) copies of survey (indicate location, and written dimensions of driveway/approach/location)
- Two (2) copies of material details (pavers, concrete, slabs etc.)
Approach: All the above plus a separate application and set of plans
- Right of way Covenant (needed for anything other than solid plain concrete)

- New Windows and Doors Affidavit signed by qualifier (not applicable for garage and shutters)

- Two 2 Copies of layout indicating location and dimensions of Windows/Doors/Garage Doors

- All rework must come either with a third set or a digital copy to submit to county

- Original / Certified job copies
- Must be reviewed and approved by Building Official (48 hours)
- Plumbing, Electrical and Mechanical sub-permits may be required prior master permit re-issuance
- All contractor information must be up to date to be able to pick up
To close expired permit:
- Will need to schedule and finalize all pending inspections
- If work is concealed prior to inspection, inspector may require a licensed engineer letter

- Must provide narrative of changes being made to Master Permit
- Permit Application with description of what is being revised.
- Application must be signed by qualifier only (if applicable)
- Two (2) sets of original signed and sealed Architectural Plans of new or revised pages only
- Original job copy and permit card not required for submittal

Closing MDC Expired Permits
1-2 Business Days
- Permit Application signed by qualifier.
- A letter from Architect/Engineer certifying that to the best of their knowledge the work was done according to the current code of that time.
- Must be reviewed and approved by Building Official.
