Volunteer Form – 2025 Health and Wellness Fair

The following form is for the 2025 Health and Wellness Fair volunteer opportunity for Saturday, March 29, 2025 from 8:00 a.m. to approximately 3:00 p.m. This form must be completed by the parent/guardian.

VOLUNTEERS:

Must be a high school student.
Max 10 sign ups.
We ask that volunteers wear comfortable shoes, use sunscreen, and wear breathable clothes on event date.
Are required to sign in AND out on event day so that they can be granted volunteer hours. Those that do not sign in AND out will not be granted volunteer hours.
