2024 Nightmare on Montrose Halloween Event Volunteer Form

The following form is for the Nightmare on Montrose Halloween Event volunteer opportunity for October 26, 2024 at Veterans Park (15151 NW 82nd Avenue). This form must be completed by the parent/guardian.

VOLUNTEERS:

Must be a middle school or high schoolstudent.
Will be required to attend scheduled evening meetings leading up to the event.
Are asked to wear comfortable shoes, and wear breathable clothes on event date.
Are required to sign in AND out on event day so that they can be granted volunteer hours. Those that do not sign in AND out will not be granted volunteer hours.
